How To Become A Client

To be considered as a client you must:

  • Be an Oregon resident or coming in for an in-person appointment. (Tabitha’s license is for OR only.)
  • Complete a brief interview (free) to make sure that we are a good match to work together.

Payment Information

Other things you may want to know: 

    • All payments are due at time of service.
    • Tabitha does not take insurance but can take FSA/HSA cards, and give you a receipt or superbill so that you can bill your own insurance (you will need to do due diligence to make sure that they accept out-of-network providers). 
    • If you are reimbursing for therapy, you will also need to check if your insurance reimburses for Licensed Professional Counselors (this is someone who is approved to counsel by the Board but is still under supervision).
    • Dr. Tabitha provides separate receipts for acupuncture services and counseling services.
    • There is a 24-hour cancellation policy (ideally 48 hours).  There is a $50 charge for last-minute cancellations.  If you are sick though, please stay home and always call in if you are going to miss an appointment.
